Maintain screen integrity
- Assess skin color, temperature, texture, moisture, mobility, edema and hydration levels.
- To assess skin lesions.
- Applying topical medicine to a wound or lesion on the skin.
- Regular dressing of wound on skin. So that healing is easy and infection can be prevented.
- Providing warm moist compression.
- Anchoring the patient to daily skin care.
- Ask the patient to use warm water while bathing and to dry the skin thoroughly after bathing.
- Avoid rubbing and friction while drying the skin.
- Then apply topical medicine to the wound or lesion.
- Applying petroleum jelly on the skin to prevent the skin from drying out.
- Change position every two hours to prevent bed sores.
Acute pain related to tissue damage or break in tissue integrity
Relieve pain
- Assessing pain level.
- Assess location, duration and intensity of pain.
- Assessing factors that trigger pain.
- Providing a comfortable position to the patient.
- Providing warm moist compression to the patient.
- Providing mind diversional therapy to the patient. So that the patient’s attention is diverted from the pain.
- Advising patients about relaxation therapy.
- Administering prescribed analgesic medicine.
Risk for infection related to loss of protective barrier of skin and mucous membrane
Prevent infection
- Assess skin integrity.
- Assessing whether signs of infection are present. Note if redness, swelling, fever is observed.
- Always keep the wound covered and do regular dressings.
- Use aseptic technique while dressing.
- Ask the patient not to touch the wound site.
- Providing streak isolation to the patient.
- Refusal to meet the patient’s relative.
- Educating patients about hand washing and personal hygiene techniques.
- Administer antibiotics prescribed by the doctor to prevent infection.
Risk for hyperthermia related to inflammatory process
Prevent hyperthermia
- To assess patient’s vital signs.
- Maintaining the patient’s room temperature.
- Providing cold application to the patient.
- If the patient feels cold, avoid cold application and provide blanket to the patient.
- Administer antipyretic drug if body temperature rises.
- Monitor temperature frequently.
Disturbed body image related to appearance of skin

- To know the patient’s attitude towards his physical appearance.
- Anchoring the patient to express his feelings and emotions.
- Allow enough time to listen to the patient.
- Helping the patient to increase his self-esteem and increase his coping power.
- Comforting the patient.
- Providing psychological support to the patient.
- To give the patient ideas to enhance his body appearance.
- Educating the patient about cosmetics and accessories so that the scar does not draw too much attention.
- Explaining the patient about the skin condition and its treatment modality.
- To provide knowledge about cosmetic surgery to patients.
Anxiety related to physical appearance of skin and prognosis
Reduce anxiety
- Assessing the patient’s condition.
- Paying attention to the psychological needs of the patient and listening carefully to the patient.
- Encouraging the patient to express his feelings, discomfort and anxiety.
- To resolve all doubts and queries of the patient.
- Providing knowledge to the patient about his condition and treatment so that his anxiety is removed and the patient becomes confident.
- Providing psychological support to the patient.
- Providing mind diversional therapy and recreational therapy to patients.
