REFERRAL SYSTEM

Sub Center or during home visit as well as P.H.C. When a patient requires further treatment and these services are not available at the centre, e.g. When specialist doctors, drugs, equipment and laboratory facilities are not available there, the patient is shifted to the nearest community health center or district hospital. What is the problem with him when referring the patient. A slip indicating the patient’s name, address, treatment given, disease etc. is given. If there is an ambulance and driver arrangement at the center, the patient should be taken in it. is referred.

Definition ::

Clinic or PHC to refer any person to a place for treatment or diagnosis. A referral note is written from and sent along with it. It is known as referral system

While referring the patient, the matter of referral should be thoroughly discussed with the head of the patient’s family or a person who can stay with the patient and is knowledgeable about the patient.

Why the patient is referred. Where to refer. Which vehicle will be available and also why it is important to refer should be explained to the patient and his relatives.

Refer Slip OR Refer Note :: –

Patient’s name, age, sex, address, date and time and referrer’s date and time, sender’s name, sender’s name and designation, name of the hospital where referral is to be made, doctor’s name, disease history in short, investigation, present complaint, given treatment .

  • Role of Nurse in Referral System

(1) Assist in referring patients.
