HEALTH TEAM

Team

A team is defined as a group of person with different levels of knowledge, abilities, skills and personality who must compliment each other and who share common goal.

Team એટલે એવી વ્યક્તિઓનું group કે જેનું knowledge, abilities, skills and personality નું level જુદું જુદું હોય છે છતાં પણ તેઓ એકબીજાના પૂરક હોય છે તેમજ તેઓના goal સમાન હોય છે.

Characteristics of team

1. Team ને ચોક્કસ હેતુ હોય છે.
2. team rules ને અનુસરે છે.
3. team પોતાની રીતે સુયોજિત હોય છે જેથી પોતાના goal પ્રાપ્ત કરી શકે છે.
4. team member એકબીજાને સહકાર આપી કાર્ય કરે છે. Health service પૂરી પાડવા health team શા માટે જરૂરી છે ?

Health service આપવા જુદા જુદા આરોગ્ય કાર્યક્રમોમાં એક વ્યક્તિ માટે બધું કાર્ય કરવું અસંભવ છે તેમજ તેમાં ઘણી આવડતની પણ જરૂર હોય છે,જુદા જુદા અભિપ્રાયની પણ જરૂર હોય છે તેથી એક કરતાં વધારે વ્યક્તિઓના મત લેવાથી target સુધી પહોંચી શકાય છે.

એક કરતાં વધારે વ્યક્તિઓ હોય તો community ની દરેક જરૂરિયાતને સમજી શકે છે અને તેવી health service આપવા health team જરૂરી છે.

Characteristics of health team

1. તેઓના હેતુ clear હોય છે.
2. ઊંચા તેમજ પ્રાપ્ત કરી શકાય તેવા goal હોય છે.
3. Health team માં manners હોય છે.
4. દરેકનો roll clear હોય છે.
5. team members growth અને કલ્યાણના અર્થે કામ કરે છે.
6. team ના દરેક સભ્યમાં કામ અને આવડતનો ઉપયોગ થાય છે.
7. team member ની કાર્ય કરવાની ક્ષમતામાં વધારો થાય છે.
8. યોગ્ય decision માટે જરૂરી છે.
9. મતભેદની apen ચર્ચા થાય છે.
10. Priority આપવાની પ્રતિભા ખીલે છે.
11. ખુલ્લાસ થી અને પ્રામાણિક વાતચીત થાય છે.
12. compromise કરવાની વૃત્તિ develop થાય છે.
13. time schedule પ્રમાણે કાર્ય કરવાની શક્તિ ખીલે છે.
14. Daily evaluation નિયમિત થાય છે.

Health team at the PHC consists of the following health personnel or staffing pattern of PHC

1. M.O (team leader)
2. block extension
3. – male or female Health supervisor
4. Health worker – male or female
5. Health guide

💞 MEDICAL OFFICER

PHC લેવલે M.O. એ team leader હોય છે પરંતુ તે એકલા હાથે community ના લોકોની જરૂરિયાત પૂરી કરી શકતા નથી, પરંતુ team ની મદદથી તે સારામાં સારી અને વધારેમાં વધારે સેવાઓ પૂરી પાડી શકે છે.

4. Training program – PHC staff ને જેમાં FHW, Helath guide, volunteer worker, angadwadi worker તેમજ dai ને training આપે છે.

💞 FHS (female helath supervisor) and FHA (female health assistant)

Regular L.H.V. ની training બંધ કરી એને છ માસની વધારાની તાલીમ આપી health assistant or helath supervisor બનાવવામાં આવ્યા.

તેઓને 20,000 ની વસ્તીમાં ચાર sub center ને આવરી લેવાના હોય છે.

એક FHW ને 5000 ની વસ્તી cover કરવાની હોય છે.

Job responsibilities of FHA or FHS

Responsibilities of female health assistant or female health supervisor (FHS

1. Supervision and guidance

• FHS staff નું supervision કરી guidance આપે છે.
• health worker ની skill અને knowledge ને મજબૂત બનાવે છે.
• health worker ને તેના program ના planning માં મદદ કરે છે.
• Weak માં એક વખત ચોક્કસ દિવસે sub center ની મુલાકાત લઇ FHW ને guidance આપે છે.
• Health worker ના કાર્યોનો periodically checking કરી મૂલ્યાંકન રિપોર્ટ PHC M.O. ને મોકલે છે.
• worker ના area માં supervisor home visit કરે છે.
• સગર્ભા માતા ને VDRL test માટે ભલામણ કરે છે તેમજ તેના રિપોર્ટ PHC M.O. ને મોકલે છે.

2. Team work

• જ્યાં જરૂર જણાય ત્યાં દરેક કાર્યમાં મદદ કરે છે.
• block level ની meeting attend કરે છે.
• Male health assistant ને તેના કાર્યોમાં મદદ કરે છે.
• PHC પર staff meeting નું આયોજન કરે છે.
• PHC M.O. ને અલગ અલગ area આરોગ્યની સેવા પૂરી પાડવામાં મદદ કરે છે.
• health program માં health team ના member તરીકે કાર્ય કરે છે.

3. Supply of equipment and maintainance of Subcenter

• health assistance ની સાથે રહીને sub center પર regular intervals એ stoke બરાબર છે કે નહીં તે check કરવું તેમજ જરૂરી સાધનો working condition માં છે કે નહીં તે જોવુ.
• દવાઓનો stoke બરાબર છે કે કેમ તેમ જ storage condition તપાસવી.
  

4. Record and report

Record ની ચકાસણી કરવી બધા જ record વ્યવસ્થિત રાખવા માટે FHS એ યોગ્ય માર્ગદર્શન આપવું અને જરૂર જણાય તો સાથે રહીને તૈયાર કરાવવા.

health worker તરફથી મળેલ report check કરીને PHC M.O. ને સુપરત કરવા.

5. Training

Health team worker ની મદદથી dai training organize કરવી અને આપવી.

આરોગ્યના અલગ અલગ category ના કર્મચારીઓ માટે પણ training નું organization કરવું અને conduction કરવું.

6. Maternal and child health

Health worker ની સાથે રહી week માં એક વખત sub center પર MCH clinic ચલાવવું જ્યારે પણ trained dai અને health worker બોલાવે ત્યારે તેમને જરૂરી સહકાર આપવો અને guidance આપવુ.

7. MTP (Medical termination pregnancy and family Welfare)

• દરેક health worker ની મદદથી weekly family Welfare clinic sub center પર ચલાવવું.
• Family planning service માટે સહકાર ન આપતા હોય તેવા કેસમાં લાભાર્થીને personally મળીને motivate કરવા.
• MTP માટેની જરૂરી માહિતી અને માર્ગદર્શન પૂરા પાડવા તેમજ યોગ્ય કેસને યોગ્ય જગ્યાએ referr કરવા.
• FHW ની મદદથી contraceptive સાધનોની વહેંચણી કરવી.
• ઘનિષ્ઠ ઝુંબેશ દરમિયાન PHC M.O. ને મદદ કરવી.
• IVCD અંગેની સેવાઓ પૂરી પાડવી.
  

8. Nutrition

Infant તથા young children માં malnutrition વાળા કેસ શોધી જરૂરી સારવાર આપવી અને જરૂર જણાય તો reffer કરવા.

9. Immunization

દરેક infant તથા ANC mother નું Immunization કરેલ છે કે કેમ તેનું નિરીક્ષણ કરવું

10.Primary health care and medical care

Minor elements ની first aid treatment આપવી.

Accident કે emergency case માં primary care આપવી અને case ને PHC કે નજીકની હોસ્પિટલમાં reffer કરવા.

Heath worker દ્વારા reffer કરાયેલા case નું follow-up કરવું.

11. Health education

અલગ અલગ વિષયો પર health education આપવું and FHW નો સહકાર મેળવવા health education માં ખાસ કરીને family planning, MCH, nutrition, immunization તેમજ blindness નો સમાવેશ કરવો.

Leader સાથે group meeting નું આયોજન કરવું તેમજ દરેક health worker ને meeting attend કરવા માટે પ્રોત્સાહિત કરવા.

Meeting દરમિયાન જુદા જુદા program ના message ની જાહેરાત કરવી અને message પહોંચાડવા મદદ કરવી.

Training નું આયોજન કરવું.

મહિલા મંડળની સ્ત્રીઓ teachers અને બીજી community ની અન્ય સ્ત્રીઓ સાથે welfare program બાબતની ચર્ચા કરવી અને તેમાં FHW ની મદદ લેવી.

12. Vital statistics

Sub center પરથી report નું collection કરી PHC M.O. ને પહોંચાડવું (monthly, weeks, annual).

💞 MALE HEALTH ASSISTANT/ MALE HEALTH SUPERVISOR

Job responsibilities of MHA or MHS

1. supervision and guidance

જેમાં.,

Male health worker તેના schedule પ્રમાણે તેના area માં visit કરે છે જેમ કે તેનું supervision કરે છે અને તે માટે અવારનવાર visit કરે છે.

MPW ના કાર્યની ચકાસણી માટે village માં ઓછામાં ઓછા 10% ઘરની visit કરે છે.

Visit સમયે malaria ની kit સાથે રાખી fever ના કોઈપણ case મળે તો તેની thick and thin slide લઈ collect કરે છે, તેમજ anti malarial treatment તરીકે આપે છે.

તેના વિસ્તાર માટે positive case radical treatment આપે છે કે નહીં તેની ચકાસણી કરે છે.

Anti malarial measures જેમ કે D.D.T. નો છંટકાવ, anti mosquito net તેમજ જ્યાખાડા હોય તો બુરી દેવા તેમજ કચરાના નિકાલ માટે લોકોને health education આપે છે.

6. Training – program નું આયોજન કરે છે અને activity related આ કામગીરીમાં તે local leader ની મદદ લે છે.

7.Communicable Disease – Communicable disease માટે પગલાં લે છે તેમજ epidemic ની પરિસ્થિતિમાં સારવાર માટે MPW ની સાથે રહે છે.

notificant case નો report main authority ને પહોંચાડે છે.

જો કોઈ હડકાયા કુતરા નો case હોય તો anti rabies vaccine અપાવે છે અને કૂતરાનો નિકાલ કરાવે છે.

Leprosy and T.B. ના case નું findout, treatment and follow-up માં મદદ કરે છે તેમજ regular treatment આપવાથી તેમને સંપૂર્ણ સારું થઈ જશે.

8.Environmental

• safe water supply
  પીવાના પાણીના સ્થાનો ખુલ્લા હોય તો તેનું ક્લોરિનેશન કરાવવું.

આ ઉપરાંત senitory laterine

• control of smoke માટે smokeless ચૂલા માટેનું construction તેમજ તેના માટે સબસીડી ની માહિતી આપવી.
  

9. UIP Universal Immunization Program

Immunization schedule પ્રમાણે 0-5 વર્ષ સુધીના બાળકોને રસીકરણ થઈ ગયેલું છે કે નહીં તેમજ pregnant ledy ને TT injection આપેલ છે કે નહીં તેની ચકાસણી કરવી તેમ જ કામગીરીનું supervision કરવું.

10. Family planning

eligible couple ને રૂબરૂ મળી family planning માટે motivate કરવા તેમજ માર્ગદર્શન કરવું.

Family planning canp નું organization કરવું તેમજ operated case નું follow-up કરવું અને લાભાર્થીને જે લાભ મળતા હોય તેના વિશે માર્ગદર્શન આપવું.

11. Nutrition

Malnourished case શોધવા અને treatment માટે M.O.ને reffer કરવા.

0-5 વર્ષના બાળકોમાં anemia, worms, Iodine deficiency, calcium deficiency, વગેરે ખામીઓ શોધવી અને તેના માટે iron folic acid વગેરે લાભાર્થી સુધી વહેંચણી કરવી અને તે તેને મળે છે કે કેમ તે ચેક કરવું.

12.Medical care

Accident કે emergency માં medical care provide કરવી અને જરૂર જણાય તો નજીકની hospital માં reffer કરવું.

13.Vital statistics

દરેક report નું collection કરી M.O. ને પહોંચાડવું.

💞 Female health worker/ Auxillary Nurse and Midwife (FHW/ANM)

5000 ની વસ્તી એ sub center પર plain area અને 3000 ની વસ્તીએ tribal અને Hilly area માં સેવા આપનાર FHW health services લોકો સુધી પહોંચાડનાર અગત્યની haelth team ની mamber છે.

Function of FHW

1.MCH care

Antenatal mother નું registration તથા સળંગ pregnancy સુધી સંભાળ લેવી.

Laboratory examination જેમાં suger, HB તેમજ ઉપરોક્ત સેવાઓ તેણી home visit દરમિયાન PHC પર કે sub center પર clinic દરમિયાન આપે છે.

Abnormal pregnancy ના case નો findout કરી hospital ને reffer કરે છે.

પોતાના area ની delivery conduct કરે છે તેમજ dai દ્વારા થયેલી delivery નું supervision કરે છે.

પોતાના area માં કરેલી delivery ની ઓછામાં ઓછી ત્રણ postnatal visit લે છે તેમજ જરૂરી સંભાળ અને સૂચનો દે છે.

Growth and development માટે જરૂરી care ની સલાહ આપે છે.

M.O. તથા health assistant ને clinic માં મદદ કરે છે.

લાભાર્થીને વ્યક્તિગત કે group માં મળતી અલગ અલગ service થી માહિતગાર કરે છે

2.Family planning

Family planning registration માટે જોઈતી માહિતી eligible couples ને પૂરી પાડે છે, Contraceptives ની વહેચણી કરે છે.

Follow up services provide કરે છે જેમાં ખાસ કરીને new married couple એ family planning ની method અપનાવેલ હોય તો તેની side effect નથી ને તેની ચકાસણી કરે છે.

Family planning સ્વીકારનાર couple સાથે, village leader સાથે, dai તેમજ health guide સાથે સારા સંબંધ કેળવી Family planning ની સેવાઓનું લોકો ને વધુ લાભ મળે તે રીતે પ્રોત્સાહિત કરે છે.

પોતાના area માં women leader ને ઓળખી તેની service માં મદદ લે છે અને મહિલા મંડળની મીટીંગ નું આયોજન કરે છે.

3.Referral services

જરૂરિયાત વાળી mother ને ઓળખી નજીકની hospital માં reffer કરે છે તેમજ ને in time સારવાર મળે તે મુજબ referral services આપે છે.

4.Nutrition

અલગ અલગ category ના group માં malnutrition ના case ને શોધે છે જેમાં ખાસ કરીને school going, young children, ANC, PNC વગેરેમાં identify કરી સારવાર આપે છે.

Iron folic acid tablet નો વિતરણ ANC, PNC તથા infant ને કરીને તેનું follow-up કરે છે.

9 month થી 5 year સુધીના બાળકોમાં vitamin-A solution નું વિતરણ કરે છે.

માતા અને બાળકને nutrition વિશેની માહિતી આપે છે અને nutrition program માં involve કરે છે.

5.Immunization

ANC mother ને TT injection આપવું.

દરેક newborn ને DPT અને OPV થી protect કરવા.

6. Comminicable Disease

home visit દરમિયાન CD ને ઓળખીને PHC M.O. ને જાણ કરવી.

malaria નાં case માં chloroquine નું વિતરણ કરવું અને smere માટેની કાર્યવાહી કરવી.

પોતાના area માં જો કોઈ વ્યક્તિ TB કે leprosy ની treatment લેતું હોય તો તે નિયમિત લે છે કે કેમ તેનું follow-up કરવું.

Diarrhea, dysentery ના કેસ હોય તો તેની treatment આપવી.

Diarrhea અને case માં ORS નું solution કેમ બનાવવું તે community માં શીખવવું અને serious case માં PHC M.O. ને reffer કરવા.

7.Dai training

પોતાના area માં delivery
કરાવતી dai નું list બનાવી તેને health and family Welfare program માં participate કરવા.

FHA ને dai training program માં મદદ કરવી.

8.Vital event

Birth and death register માં પોતાના area ના birth અને death ની નોંધણી કરવી.

9.Record and report

• ANC નું Registration કરવું.
• FHA ને તેના record માટે મદદ કરવી.
• Report બનાવવાના હોય તો સમયસર બનાવવા અને health assistant ને સમયસર submit કરવા.
• પોતાના area નો map બનાવી service દરમિયાન તેનો ઉપયોગ કરવો.

10.Primary medical care

• minor elements ની treatment
• accident કે emergency માં treatment
• Serious case ને reffer service પુરી પાડવી.

11.Team activity

PHC પર staff meeting નું આયોજન કરવું અને block level ની meeting attend કરવી.

MHA ને મદદ કરવી.

દર અઠવાડિયે જરૂર જણાય ત્યાં માર્ગદર્શન માટે આયોજન કરવું.

sub center ની ચોખ્ખાઈ જાળવવી.

Camp ના આયોજનમાં મદદ કરવી.

Team ના દરેક સભ્યો સાથે જરૂર જણાય ત્યાં દરેક રીતે મદદરૂપ થવું.

💞 MALE HEALTH WORKER/MULTI PURPOSE HEALTH WORKER

Function of MPW

1. Conductor survey of the sub Centre area and maintainance record of all family
2. Vital statistic
3. participate in malaria control program
4. participate in leprosy Control Programme
5. participate in family planning service and provide information on the family planning method
6. reporting about communicable disease
7. co-ordinate the activities with health worker
8. maintain record

💞 District public health nurse (DPHN)

position

તેણીનો job નો area district છે અને district માં સ્વતંત્ર કાર્ય કરે છે.

બધા જ nursing person કે જે પોતાના જિલ્લાના field area માં જેવા કે primary health center, sub center, family Welfare program તેમજ અન્ય health program માં તેણીની જવાબદારી હોય છે.

Technical matters માં તેણી health director ને કે health services ના nursing director ને contact માં રાખે છે તેમજ દરેક Technical બાબતે તેમજ health ને લગતા program બાબતે તેઓની પાસેથી guideline મેળવે છે.

Function

1. General duties – પોતાના જિલ્લામાં free services તથા community health ની દરેક service ને વિકસાવે છે, તેમજ જરૂરી માર્ગદર્શન આપે છે અને તેનું organization કરે છે.

પોતાના જિલ્લામાં health ને લગતી સેવાઓના discussion માં ભાગ લે છે.

જિલ્લા પરિષદમાં DHO ને midwife ની સેવાઓની તથા nursing ની સેવાઓ ની જરૂરિયાત વિશે જાણકારી આપે છે તે માટેની વાતચીત કરે છે.

2.Administrative duties – જિલ્લામાં nursing and midwife ને લગતી સેવાઓના program માં તથા program ની policy માં સુધારા માટે implementation કરે છે જે તેણીની જવાબદારી છે.

Nursing staff નાં appointments, transfer, leave, further education તેમજ selection માટેના સૂચનો DHO ને આપે છે.

Supplies તથા equipment વગેરેની જરૂરિયાત બાબતની જવાબદારી નિભાવે છે.

બજેટ planning માં participate થાય છે.

જિલ્લામાંથી આવતા reports નું મૂલ્યાંકન કરી ને DHO office માં મોકલે છે.

પોતે તૈયાર કરેલા માસિક, ત્રિમાસિક છ માસિક, અને વાર્ષિક report DHO ને મોકલે છે.

જિલ્લામાં આપવામાં આવતી Nursing services નું planning કરે છે અને conduct કરે છે.

3. Supervisor – પોતાના જિલ્લાના community health nursing ના કર્મચારીઓના supervision માટે જવાબદાર છે.

કર્મચારીને પોતાનામાં રહેલી શક્તિના વિકાસ માટે પ્રોત્સાહિત કરે છે, જેથી કરીને community માં તે સારામાં સારી care આપી શકે.

Health team ના દરેક member નો ઉત્સાહ જાળવવામાં મદદ કરે છે, તેમજ health team ના આ group સાથે રહી લોકોની સેવામાં assist કરે છે.

દરેક કર્મચારીને સોંપવામાં આવેલ કાર્યનું વ્યક્તિગત મૂલ્યાંકન કરે છે.

4. Educational work –
   Nursing person ના orientation training program માં continues education અને refresher course માટે તે જવાબદાર છે.

ઉપરોક્ત in service education માટે પણ તેની જવાબદારી છે.

Nursing person માં refresher course તથા બીજા short term course નું planning કરે છે.

Team ના બીજા member ને મદદ કરીને non nursing person માટે training program નો વિકાસ કરવો.

Health worker ને તેમના rogram માં મદદ કરવી અને માર્ગદર્શન પૂરું પાડવું.

Staff ને આગળ education લેવા સૂચન કરવું, જેમાં જિલ્લામાં, જિલ્લા ની બહાર, state ની બહાર કે પછી વિદેશ હોય.

nursing સેવાઓ ની માહિતી community તથા staff ને આપવી.

Rural feild ના health center માં વિદ્યાર્થીઓને મુકવામાં આવે ત્યારે તેઓને training આપે છે.

In-service education program or job training

Institute ના staff ને development માટેના તેમજ patient care ની quality improve કરવા માટે આ training આપવામાં આવે છે.

આ Training ના AIMS નીચે મુજબ છે:

1. તેમના member ની skill develop કરવા માટે
2. team member ને વધારાની જવાબદારી સ્વીકારવા તૈયાર કરવા માટે
3. knowledge વધારવા માટે
4. agency ની policy, philosophy તેમજ તેના goal ને સમજવા માટે Advantages
5. સતત education તથા નવુ knowledge મળી શકે છે.
6. Skill develop થાય છે અને moral power વધે છે.
7. teaching ability વધારવામાં મદદ કરે છે.
8. સારા supervision માટે મદદ કરે છે.
9. વ્યક્તિગત profession development માટે મદદરૂપ થાય છે.
10. IPR improve રવામાં મદદરૂપ થાય છે.

Supervision of health person

એટલે જે કોઈ કાર્ય કરવામાં આવતું હોય તે કાર્યને ઉપરના લેવલે થી ચકાસવું.

તે કાર્યમાં ક્યાં ઉણપ છે, ક્યાં ભૂલો છે તે કામગીરીનું checking કરી તેને સુધારવાના પ્રયત્નો કરવા એટલે supervision.

અત્યારના સમયમાં supervision એ એક guide તરીકે કરવામાં આવતી કામગીરી છે, જેમાં technical guidance, advise, demonstration and training નો સમાવેશ થાય છે જેથી વ્યક્તિનું કાર્ય કરવાનું interest વધે છે અને કામ સારી રીતે થઈ શકે છે.

Supervision એ skill develop કરવા માટેનું પણ અગત્યનું factor છે.

Supervision હંમેશા leadership ની અપેક્ષા રાખે છે.

Supervisor એ હંમેશા good teacher છે.

સારા supervision observation થી થઈ શકે છે.

Indirect supervision માં record તથા report તપાસવામાં આવે છે.

💞 COMMUNITY HEALTH NURSE

Role of CHN in supervision

Nursing service ની સાથે community health nursing માં supervision એ અગત્યનું પાસું છે, જે દરેક health program માં જોવા મળે છે.

CHN ના supervision માં maternal health ની સાથે infant, preschool child તેમજ CD control નો સમાવેશ થાય છે.

સામાન્ય રીતે qualified nurse ને હંમેશા supervisor તરીકે નિમવામાં આવે છે.

તેણી કર્મચારીઓ માટે training, administration તથા teaching જેવી બાબતોને આવરી લે છે.

વિશ્વના દરેક ભાગમાં CHN એ એક નમૂના રૂપ વ્યક્તી છે તેથી તેણીનો ROLL નીચે પ્રમાણે વર્ણવી શકાય:,

1. Health center

Sub center તથા health department માં તે nursing person જેમકે, ANM, FHW વગેરેના supervision માટે જવાબદાર છે.

તેણીએ health center, sub center કે community level પર સેવાઓ આપવાની હોય છે.

આરોગ્યની સેવાઓનું આયોજન, નિયમિત અને સંકલન કરવાનું હોય છે.

Community need assessment કરી તે માટેની સેવાઓનું આયોજન કરી તે સેવાઓમાં participate થવું અને માર્ગદર્શન પૂરું પાડવું.

વ્યક્તિગત કે field માં direct તથા indirect સેવાઓ આપવા માટે તે જવાબદારી નિભાવે છે અને supervision કરે છે.

Function of CHN in PHC

CHN નું work, તે કંઈ post ઉપર work કરે છે, તે કંઈ જગ્યાએ કામ કરે છે તેમજ.., તેનું education અને experience પ્રમાણે અલગ અલગ હોય છે.

ભારતમાં CHN private agency તથા PHC માં supervision અને instruction આપવાનું તેમજ બીજા અગત્યના કામ પણ કરે છે.

ખાસ કરીને, PHC લેવલે તેનો રોલ આ પ્રમાણે છે:

1. Assessing

A. Study or identification of community : આમાં ખાસ કરીને community નો અભ્યાસ કરવામાં આવે છે, જેમાં ખાસ કરીને population,ભૌગોલિક પરિસ્થિતિ, occupation of people, social and economical condition, culture and beliefs તેમજ વિચારસરણી અને આરોગ્ય ની જરૂરિયાતને ઓળખે છે.

આ ઉપરાંત આરોગ્યની જરૂરિયાતને ઓળખવા માટે તેઓ problem નું survey કરે છે અને તેના પરથી સેવાઓનું આયોજન કરે છે.

B. Analysis: જરૂરી ક્ષેત્રની માહિતી મેળવી અભ્યાસ કરી તેનો અંદાજ કાઢે છે તેમજ તેનો રિપોર્ટ પોતાના કાર્ય સ્થળ પર આપે છે પોતાના કાર્યના અનુસંધાને તેના એરિયામાં કઈ કઈ સેવાઓ ઉપલબ્ધ છે તેનું અંદાજ કાઢે છે.

2. Planning

Comprehensive nursing care માટે વ્યક્તિગત, કૌટુંબીક કે community ના માંદગી દરમિયાન તેમજ health ને લગતી services નું planning કરે છે.

કુટુંબની સાથે રહીને, medical team ની સાથે રહીને, community development project ના member ની સાથે રહીને planning કરે છે.

School health services, clinic and health center માટે planning કરે છે.

Health education program માટે કે community ને લોકોની જરૂરિયાત માટે planning કરે છે.

3. Implementation

• જેમાં patient ને direct care આપે છે, જેમાં family ને community માં care પૂરી પાડે છે,
• Doctor ને early diagnosis માં મદદ કરે છે, તેમજ તે માટેની સૂચનાઓમાં અમલ કરે છે જેમકે, injection, dressing, investigation વગેરે..
• ઘરમેળે જે કંઈ સુવિધા ઉપલબ્ધ હોય તેના દ્વારા adequate care પૂરી પાડે છે.
• Family માં માંદગી દરમિયાન માર્ગદર્શન પૂરું પાડવું તેમજ જે કોઈ સારવાર આપવામાં આવતી હોય તેનું supervision કરવું.
• Health teaching આપવું અને family માં counseling કરવું.
• જરૂરીયાત પ્રમાણે યોગ્ય સેવાઓનો લાભ મળે તે માટે સંદર્ભ સેવાઓની માહિતી આપવી.
• Family ને જે કોઈ જરૂરી services આપી હોય તેનો વ્યવસ્થિત recort કરવો.
• Collection of vital statistics
• દરેક પ્રકારની services ની information community ને પૂરી પાડવી.
• ANC નુ registration કરવું
• newborn care આપવી અને postnatal care આપવી વગેરે…
• supervision and guidelines community ન health worker ને પૂરું પાડવું અને જરૂરી સૂચનાઓ આપવી.

4. Evaluation

• પોતે જે કામગીરી કરે છે તેનો monthly report તૈયાર કરવો અને તેનું analysis કરવું.
• clinic માં જે નવા case આવતા હોય તેની નોંધણી, તેની revisit ની ચકાસણી તેમજ તેની દરેક visit નું મૂલ્યાંકન કરવું.
• survey resource અને revisit માં participate કરવુ.
