Level of health care:

To define the health care:

Health care ના agent અથવા professionals દ્વારા આપવામા આવતી Multiple services or multitude services એ health ને promote કરવા,observe કરવા અથવા restoring of health  કરવાના હેતુથી individual, family & community માં health care provide કરવામાં આવે છે.

તેથી, આપણે કહી શકીએ કે prevention, diagnosis, treatment ,promotion, rehabilitation and health education એ health services ના મુખ્ય ઘટકો છે.

* mortality and morbidity rates ઘટાડવા

* life expectancy વધારવા

* nutritional status improve કરવા

* basic sanitation provide કરવા

* manpower and other resources નું development કરવા

Health care માં ઘણી વિશેષતાઓ હોય છે જેમાં કેટલીક આ પ્રમાણે છે;-

1. Appropriateness (Relevance)  [યોગ્યતા (પ્રાસંગિકતા)]: health care એ health need નીpriorities and policies અનુસાર હોવી જોઈએ.

2. Comprehensive- એટલે કે – preventive, curative, or therapeutic and promotional services નું શ્રેષ્ઠ મિશ્રણ છે.

3. Adequacy: એટલે કે જો સેવાઓ જરૂરિયાતો માટે પ્રમાણસર હોય.

4. Availability:- એટલે કે administrative unitsની વસ્તી અને health facility વચ્ચેના rate (ex..Population per center, Dr- population rates)

5. Accessibility: એટલે કે geographic accessibility, economic accessibility or cultural accessibility હોઈ શકે છે.

6. Affordability: એટલે કે Health care ની કિંમત individual અને stateમાં હોવી જોઈએ.

7. Feasibility: એટલે કે અમુક  procedure ની effectivness, logistic support, main power and material resources હોવા જોઈએ.

The services must be based on desired standards:- (સેવાઓ ઇચ્છિત ધોરણો પર આધારિત હોવી ): –

well- organized care પૂરી પાડવા માટે જવાબદાર હોય તેવા લોકોને professionals knowledge ની જરૂર છે.

Health care services સામાન્ય રીતે three stage માં ગોઠવવામાં આવે છે.

દરેક stage એ higher level દ્વારા સમર્થિત છે કે જયાં patient ને refer કરવામાં આવે છે.

1. Primary health care

2. Secondary health care

3. Tertiary health care

આ individual વ્યક્તિ અને health system વચ્ચે contact નું first stage છે, જ્યાં “essential – આવશ્યક” health care (primary health care ) પૂરી પાડવામાં આવે છે. મોટાભાગની prevailing (પ્રવર્તમાન) health problems અને complaints (ફરિયાદો) નો આ stage એ સંતોષકારક રીતે નિકાલ કરી શકાય છે. આ stage લોકોની સૌથી નજીક છે. Indian context (સંદર્ભ) માં, આ care એ primary health centre અને ત્યાંના subcentre દ્વારા community માં care પૂરી પાડવામાં આવે છે

આ heath care એ Disease ના prevention પર વધુ ધ્યાન આપે છે.

આ stage વધુ complex problems (જટિલ સમસ્યાઓ) નો સામનો કરવામાં આવે છે આ care માં આવશ્યકપણે curative services નો સમાવેશ થાય છે. આ care એ district hospital and community health centers દ્વારા પૂરી પાડવામાં આવે છે.

આ stage ની care એ health system માં first referal level  (FRU- first referal unit) તરીકે છે.

આ stage એ સુપર speciality care provide કરે છે.

આ care એ regional/ central level institutions દ્વારા પૂરી પાડવામાં આવે છે.

Health care services પૂરી પાડવા સિવાય planning, management and research work પણ આ stage એ કરવામાં આવે છે.

Health workers  માટેના મોટાભાગના educational and training programes પણ આ stage એ હાથ ધરવામાં આવે છે.

વધુમાં,tertiary stage એ primary level એ હાથ ધરવામાં આવેલી ક્રિયાઓને supports and compliments આપે છે.

Role of nurses in referral system:

Definition:

Referal system માં આવશ્યક quality care પૂરી પાડવા માટે Patient ને lower level or less specialty centre માંથી higher level or super specialties center માં refer કરવામાં આવે છે આ system ને referal system તરીકે  વ્યાખ્યાયિત કરી શકાય છે.

Referral system માં nurse નો main role…

* તમામ individual nurses ને referal system વિશે જાણકારી હોવી જોઈએ.

* patient ની seriousness અનુસાર તેની categories ની જાણ હોવી જોઈએ.

* referal system માં તેમની પોતાની limitations and responsibilities થી વાકેફ હોવા જોઈએ.

* emergency ના case માં, patinet નો જીવ બચાવવાનો પ્રયાસ કરવો જોઈએ અને પછી જ તેને refer કરવું જોઈએ.

* referral slip અથવા form એ correctly and accurate રીતે fillup કરવું.

* serious patient ને refer કરતી વખતે life

saving drugs, articles ને સાથે ગોઠવવા જોઈએ.

* જો facilities available હોય તો Telephone, Television, network, medical transcription or Telemedicine etc દ્વારા patient માટે consultation મેળવી શકાય છે.

* જો શક્ય હોય તો patient સાથે જાઓ.

* યોગ્ય documentation કરવું.
