pen relieving drug

• સુવાવડ નો દર્દ ખૂબ જ અસહન હોય છે. આ પેઈન માતાને લેબર શરૂ થાય ત્યારથી તેણી કલાકો સુધી તેમાંથી પસાર થાય છે. પેઈન ના લીધે માતા ચિંતાતુર થઈ જાય છે તેને ખાવામાં પણ રસ હોતો નથી. આવી કન્ડિશનમાં તેને પેન ઓછું થાય તેવી કોઈ મેડિસિન આપવામાં આવે તો તેને આશીર્વાદરૂપ થઈ શકે.

Injection morphin

• morphin એ marcatic ગ્રુપની ડ્રગ છે જે CNS ને ડિપ્રેશ કરે છે. ( Enteral nerve system ) ને ડિપ્રેશ કરે છે. જેથી પેઈન ઓછું થાય છે. મોરફીન ડ્રગ માર્કેટ છે. એટલે તેનો ઉપયોગ બહુ જ સાવસેતી પૂર્વક કરવો. લેબર સમયે બેબીના જન્મ પહેલા ચાર કલાક અગાઉ આપવામાં આવે છે.

Dose

• morphine એ પેશન્ટના વાઈટ અને તેને જનરલ કોન્ડિશન પ્રમાણે ૧૦-૧૫mg આપવામાં આવે છે આ adult drug dose છે.

Route

• orally IM કે IV અપાય છે.
  Action of drug
• morphine આપ્યા પછી પેશન્ટમાં શક્તિનો સંસાર થયો હોય તેવું લાગે છે. Mild પ્રકારના hypnotic અસરો જોવા મળે છે જેમ કે ઘેન આવે છે. પેશન્ટ સૂઈ રહે છે.
• surgical emergency હોય ત્યારે preanesthetic drug તરીકે તેનો ઉપયોગ કરવામાં આવે છે તેમાંથી pupil contract થાય છે.
• મોરફીન લેનાર વ્યક્તિને વારંવાર લેવાથી વ્યસન થઈ શકે છે આ ડ્રગ્સને ઈફેક્ટ ચાર કલાક સુધી રહે છે.

Contraindication

• Baby ને asphexia history હોય ત્યારે
• માતા એનેમિક હોય ત્યારે
• maternal distress હોય ત્યારે.
• foetus distress હોય ત્યારે.
• labour 2nd stage માં.

Side effects

Nursing care

• આના ડોસ ડોક્ટરના ઓર્ડર પ્રમાણે સ્મોલ ડોઝ આપવામાં આવે છે તથા તે આપ્યા બાદ પેશન્ટ સાથે સતત રહેવું જોઈએ ‌
• પેશન્ટને એકલું છોડવું નહીં
• રેસ્પીરેટરી distress થવાની સંભાવના છે તેથી પેશન્ટ અને ચોખ્ખી હવા મળે તેવું વાતાવરણ રાખવું.
• vital sing check કરવા.
• bowel movement જોવા.

• આ pain killer drug છે. તેની અસર anti inflammatory and pain reliever એમ બે રિતે જોવા મળે છે. આ drug primary dismenonrrhea વખતે પણ અપાય છે.

• abdominal TL
• spinal anaesthesia આપતા પહેલા LCS મા જે પેશન્ટમાં જનરલ એનેસ્થેસીયા કોન્ટ્રાકશન હોય તેવા પેશન્ટમાં એનેસ્થેટિક એજન્ટ તરીકે અપાય છે.

1. Oxytocin

Action

• તે યુટ્રેસના કોન્ટ્રાકશન અને cervix ના dialatation મદદ કરે છે.

Rout & dose

• I/V 5% glucose નાં pint મા કે 500ml kl નાં pint માં drip method દ્વારા આપી શકાય છે.
• labour ના indication માટે 2.5 થિ 5unit pint મા આપી શકાય છે.
• ક્યારે abortion , PPH K LSCS માં 10 to 20 unit pnit માં આપવામાં આવે છે.
• I/M shoulder ની delivery પછી 5 unit oxytocin I/M આપવામાં આવે છે.

Ergot Derivatives ( એગોટ ડેરીવેટીવ )

• આ group માં મુખ્યત્વે નીચે મુજબની બે ડ્રગ્સ વપરાતી હોય છે.

Ergometrine / methergine

Action

• તે uterus ના કોન્ટ્રાકશન અને cervix ના dilation મા મદદ કરે છે.

Dose

• dose એ માતાની કન્ડિશન પર આધાર રાખે છે. અને I/V , IM કે oraly આપી શકાય છે. Ampal માં IM solution આવે છે અને તેનું પ્રમાણ 0.2mg હોય છે.
• તે methyl ergometrine ના ompul સ્વરૂપે આવે છે.

Indication

• PPH ને prevent કરવા.
• PPH ની treatment માટે.
• first abortion bleeding ને અટકાવવા.
• MTP વખતે
• Hysterectomy વખતે.

નોંધ:- આ ઇન્જેક્શન interior shoulder બહાર આવી જાય ત્યારબાદ IV આપી શકાય છે પરંતુ general placenta ની delivery થયા બાદ અપાય છે.

• આ ઇન્જેક્શનને foetus delivery થાય તે પહેલા આપવામાં આવે છે તો uterus નો કોન્ટ્રાકશન વધી જવાના કારણે uterus rupture થઈ શકે છે અને foetus dath પણ થઈ શકે છે.

• તે cervix ના dilation કરવા માટે.
• પ્રેગનેન્સીને ટર્મિનેશન કરવા માટે
• molar pregnancy ને terminate કરવા માટે.
• PPH ને અટકાવવા તેમજ તેની ટ્રીટમેન્ટ માટે.

1. Magnesium sulphate ( mgso⁴ )

Action

• anti conversion and towlytic agent તારીકે વર્તે છે.

Dose

Loading dose

• 1 ampule અંદર 2ml solution આવે છે જેની અંદર 1gram mgso⁴ હોય છે.
• I/U આપવા માટે mgso4 કરીને 15 થી 20 મિનિટ સુધી આપવું.
• I/M માટે બંને buttocks પર 5gm mgso4 puch કરવો એમ total 10 gm I/M આપવામાં આવે છે.
• 15 મિનિટમાં કન્વર્ઝન ફરી આવે તો 2gm mjso4 dilute કરીને I/V આપવું.

Maintanamce dose

• દર 4 કલાકે 5gm mjso4 I/M આપવું.
• I/V 1 TO 2 gm hour આપવું.

2. Diazepam

• ડ્રગ્સ ને થેરાપ્યુટિક ઇફેક્ટ ઈવાલ્યુએટ કરવું.
• બ્લેડિંગ માટેના સાધન જોવા
• પેશન્ટના ફેમિલી ને સમજાવું કે વધારે પડતો બ્લોડ લોસ , હેડ એક , નોજીયા, વોમીટીંગ હોય તો ડોક્ટરને જાણ કરવી.
• TPR BP રેગ્યુલર એસેસ કરવા.

Role of nurse in prosthagindin

• રેસ્પિરેટરી ટ્રેક રીધમ અને ડેપ્થ ઊંડાણ અસેસ કરવી.
• વઝાઈનલ ડિચાર્જ , ઈચીંગ , ઈરીટેશ અને ઇન્ફેક્શન અસેસ કરવું.

• કેલ્શિયમ ગ્લુકોનેટ અવેલેબલ હોવું જોઈએ જેથી mgso4 ની ટોક્સિસિટી ટ્રીટ કરી શકાય.
• કન્વર્ઝન દરમિયાન ક્લાઈન્ટ ને સિંગલ રૂમમાં રાખવું.
• ક્લાઈન્ટને લેફ્ટ લેટરલ પોઝિશન આપવી.
• રેસીપી ડિપ્રેશન રેટ 12 મિનિટ કરતાં ઓછો હોય તો ડ્રગ બંધ કરવી
• ડીલેવરી ના 24 કલાક પહેલા ડ્રગ આપેલ હોય તો ફીટસ ના રેસ્પિરેશન રેટ અને રીધમ ડીલેવરી બાદ ચેક કરવા.
• પેશન્ટના રિલેટિવ ને એક્શન , સાઈડ ઇફેક્ટ અને લક્ષણો વિશે સમજાવો.
• IV ઇન્કયુઝન દરમિયાન પેશન્ટને બેડ પર રાખવું.
